An 85-year-old man presents with acute confusion. He has recently been started on diuretics by his GP when he presented with presumed cardiac failure. Two days prior to his admission his son noted that he had a productive cough and was increasingly breathless. He has a past medical history of lung cancer and is on regular analgesics.
